Required Documents for Free Tablet Approval
To get approved, you must prove your identity and eligibility. Use this guide to gather the right documents before you apply.
Why Documentation Matters
The National Verifier (USAC) requires proof that you are who you say you are and that you qualify for the program. Submitting clear, valid copies of these documents helps you avoid rejection and delays.
1. Proof of Identity
You must provide ONE unexpired official ID:
- Driver's License (U.S.)
- Passport or Passport Card
- State ID Card
- Tribal ID Document
- U.S. Military ID
2. Proof of Address
If your ID doesn't show your current address:
- Utility Bill (Water, Gas, Electric)
- Lease or Mortgage Statement
- Current Driver's License
- Medicaid/SNAP Benefit Letter
- Tax Return (W-2 or 1040)
3. Proof of Eligibility
You only need to prove eligibility through ONE method: Program Verification OR Income Verification. Do not send both unless asked.
Program Participation
Submit a benefit award letter or statement from the current year for:
- SNAP (Food Stamps) Approval Letter
- Medicaid ID Card or Letter
- SSI Award Letter
- Federal Public Housing Assistance Lease
- Veterans Pension Benefit Statement
- WIC Benefit Verification
Low Income Proof
If you don't use a program, prove your annual income is under 135% of guidelines:
- Prior Year’s State or Federal Tax Return
- Current Pay Stubs (3 consecutive months)
- Unemployment/Workers' Comp Statement
- Social Security Statement of Benefits
- Divorce Decree or Child Support Award

Never submit fake or altered documents. The National Verifier system uses automated databases to check your information. Submitting false documents constitutes fraud and can lead to a permanent ban from government assistance programs.
Only apply through official providers or the National Verifier website. legitimate programs will NEVER ask for payment to "reserve" a free tablet.
